Phase 1:
Step 1: Fill out the Pre-Qualification Questionnaire which takes the average clinic 20 minutes or less to complete.
Please Note: Our team will review your questionnaire, and most clinics receive an approval or denial within minutes. If you don’t receive an email within an hour, it usually means we need additional information. In that case, our team will reach out for clarification within 72 hours.
Step 2: Pre-qualified clinics can complete the final approval process by signing the agreement documents provided after questionnaire submission which takes approximately 5 minutes.

Phase 2: Get In-Network
Pay the application fee to unlock Tier 1.
Complete Tier 1 Certification Course (30-45 minutes)

Phase 3: Grand Plan Proposal
Submit payroll census
Review proposal
Determine effective date
Sign and submit contract package

Phase 4: Plan Setup
Set up open enrollment meetings with employees
Employees sign and submit opt-in forms
Submit COI with Align & Co. as additional insured, proof of malpractice for all doctors, copy of active license, signed service agreement, W-9, and payment info.

Phase 5: Plan Effective Date
The effective date starts on the 1st day of the month following the open enrollment period when you can begin providing services for your staff under The Grand Plan.

Phase 6: 1st Payment
Your first payment will be issued 65 days from the effective date of the plan (subject to the employer premium received.)
After the first payment, recurring payments will be received on the 28th of each month.
